The following is a list of admission requirements:

1. An application form shall be completed and returned to the Center office.
2. Must be accompanied with the yearly Registration Fee
3. Must have a copy of the Immunization Compliance Form 121 that can be secured from your family physician, pediatrician, or local health department.
4. When enrolling a child from our waiting list, preference will be given to resident members of Northwood Church.
5. When enrolling a child from our waiting list, children who have siblings currently enrolled in the Center will be given preference.
